IMPORTANT NOTICE
THIS DOCUMENT INCLUDES A RELEASE AND WAIVER OF LIABILITY, INCLUDING FOR ORDINARY NEGLIGENCE. PLEASE READ CAREFULLY BEFORE SIGNING.

Assumption of Risk
I understand and acknowledge that boxing is a physically demanding contact sport. Participation in boxing training, sparring, conditioning, drills, pad work, strength training, and related activities involves inherent risks, including but not limited to cuts, bruises, sprains, fractures, concussion, head trauma, permanent injury, paralysis, or death.
I acknowledge that these risks cannot be eliminated regardless of instruction, supervision, or equipment. I voluntarily choose to participate, or allow my child to participate, with full knowledge and acceptance of these risks.

Transportation Acknowledgment
I understand that participation may require travel to other gyms, training locations, or events. Transportation may be provided by:

Coach Louis Bell, Authorized Cornerstone Gym staff or agents, Personal vehicles
Rented vehicles, if necessary

I acknowledge that such transportation is incidental to participation and consent to riding as a passenger. I understand that travel carries inherent risks separate from athletic participation.

Release and Waiver of Liability
To the fullest extent permitted by Maryland law, I hereby release, waive, discharge, and hold harmless KS Athletics LLC (d/b/a Cornerstone Gym), Coach Louis Bell, its owners, members, managers, employees, coaches, agents, contractors, volunteers, affiliates, and any associated facilities from any and all claims, demands, causes of action, or liability of any kind arising out of or related to participation in Cornerstone Gym activities or transportation, including claims based on ORDINARY NEGLIGENCE.
This release applies to all known and unknown injuries or damages and is binding upon myself and my heirs, executors, administrators, and assigns.
This release does not apply to acts of gross negligence or willful misconduct, as defined under Maryland law.

Indemnification (Minors)
If signing on behalf of a minor, I agree to indemnify and defend KS Athletics LLC and all released parties against any claims brought by or on behalf of the minor arising from participation in Cornerstone Gym activities or transportation, to the fullest extent permitted by Maryland law.

Medical Responsibility
I represent that I, or my child, am physically and medically able to participate. I understand that Cornerstone Gym does not provide medical care and that I am solely responsible for all medical expenses incurred as a result of participation. I agree to disclose any medical conditions or limitations that may affect safe participation.

Continuing Agreement
This Agreement is intended to be a continuing and ongoing waiver, remaining in effect for all current and future participation unless revoked in writing. If any portion is deemed unenforceable for minors, it shall remain valid to the maximum extent permitted by law and continue upon the participant reaching the age of majority unless revoked in writing.

Governing Law and Venue
This Agreement shall be governed by the laws of the State of Maryland. Any legal action shall be brought exclusively in a court of competent jurisdiction within the State of Maryland.

Severability
If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.
